Overview

There are basically 4 main reasons for conducting an incident investigation, they are:
  • To establish the immediate and underlying cause of the incident.
  • To prevent recurrence of a similar incident
  • To comply with company and legal requirements
  • To prevent further unsubstantiated claims
Accidents very rarely happen as a result of a single cause. There are many factors that have to be considered for an investigation to be effective. Unfortunately, site investigations can be traumatic, especially if the incident has resulted in a fatality; at best there will be panic and confusion, people running around in a state of shock, physical evidence can be destroyed very quickly.

The investigator still has to conduct an investigation.

This course is intended to assist people who’s job it may be to conduct an investigation.
